Although not included anywhere in the packaging, here are the sources for the tracks:
Tracks #1-10, from 1973 World Tour (Same tracks as tracks 1-10 on Too Late to Stop Now Outtakes)
Tracks # 11-14, 5/26/73, Los Angeles, CA (Same tracks can be found on Wild Night)

Jewel case inside cover and CD back cover is the exact artwork from Astral Weeks commercial release.

This boot is a clone of another by the name of Wild Night (or vice versa).

Art Siegel voices doubts:
I think it's highly unlikely that the final 4 cuts are from 5/26/73. If live, they are almost certainly from 6/74 or after. If 11, 12 and 14 are studio versions, they are listed in Philosopher's Stone as '74 Holland and '75 Sausalito.
